In a word NO. I've said on a previous thread elsewhere that from a discussion during a UKAS assessment it became clearer to me that TRUE preventive action should come from an initiative or even an observation but not necessarily from an incident ... that is more likely to be a remedial and corrective action.
For something to be truly a preventive action it hasn't happened yet and involves some crystal ball gazing ... asking what if? but as always more things can happen than actually do.
