I've recently purchased the above book and found it very useful, particluarly when investigating the FAA's approach to legislation.

Have any other forum users had a read or can recommend any other reading?

Sorry for extremely long delay but.....

My bible is "Managing the risks of organisational accidents" by James Reason.  I am not alone in using this as a benchmark read.  Its is published by Ashgate or you can buy from Amazon.com which I did recently as my old copy was lost/nicked.   I find in pure safety terms you don't need much else.  If you want of focus on risk or human behaviour there are more specialist books but as a good all rounder written by the expert himself, I have yet to see anything better.

Hi Andrew,

I'm sure we've got a copy of that somewhere in the office, but Reason's 'Swiss Cheese' model is very well known to me and many other within the sector I'm sure.

Swiss cheese is old hat.  The book is more about organisational risks/latent failures and SMS in general.    Do you have access to ICAO doc9859 (think that's the number) which has just been issued in June as the second edition on SMS.  Its a very thick document but easily read and is now the aviation safety bible as it is a collection of contracting states best practice.  Sorry if I am giving you info you already know.  I have seen the above book advertised but have not purchased it.  I also recently purchased "No Excuses" operational risk management and business processes.  Good I think (1/2 way through) but heavy going.

I'm reading a good book entitled "Just Culture - Balancing Safety and Accountability" - it's a very good read, specifically in relation to reporting systems. Also, it's not aviation-specific and looks at a number of other sectors, including healthcare and the nuclear industry.
